To apply for Arts Access, we require proof of eligibility in the form of a photograph or scanned copy of a valid/current benefit card. The form below will include an opportunity to upload a file or you can print and mail a copy to our office.

 

Please note: If you are applying on behalf of an eligible individual, complete the application under their name, not yours. We cannot complete registration if the attached proof of benefits does not match the applicants name.
